Who We Are

A complete team of caring individuals comprised of primary care doctors, nurses, social workers, home care coordinators, recreational therapists, physical therapists, home health aides, nutritionists, drivers, nursing assistants, services specialists, day center managers, medical specialists, and volunteers.

What We Do

We provide all aspects of healthcare for our elderly community allowing them to live in the safety and comfort of their home instead of being placed in a nursing facility. Our services cover doctor's appointments, medication, transportation, socialization, physical therapy, and so much more all in one location making if comfortable and easier for them to get the care they need. "The mission of Hope PACE is to provide client-centered holistic long-term care to frail elderly individuals while supporting their core desires for dignity, autonomy, and the ability to maintain a degree of independence."
